The only gift a mother in law would like is for you to give her her son back. With that I mean: make them a reservation at a coffee shop/restaurant etc and let him spend some quality time together.

I know you said not to spa treatmetns but consider a mani and pedi, my mother in law also has everything and I booked one for her birthday, she absolutely loved it!
